The Community of Madrid will invest 38 million euros annually to improve compensation for on-call shifts, nights, and holidays in the SERMAS starting in 2028. A doctor will go from earning 424.83 euros for a weekday on-call shift to 566.61 euros, a 33% increase.
The president of the Community of Madrid, Isabel Díaz Ayuso, announced on Monday a new salary increase for doctors and nurses who work on-call shifts, nights, and holidays in the Madrid Health Service (SERMAS). The measure will come into effect on January 1, 2028 and will involve an investment of 38 million euros annually, as she revealed during the first day of the Debate on the State of the Region, reports miracorredor.tv.
The increase adds to the 70 million euros already allocated to improve these compensations during 2026 and 2027. With this new injection, the Community of Madrid positions Madrid's doctors as the highest paid in Spain for hours worked on-call, according to the regional government.
After allocating 70 million euros to increase compensation for on-call shifts, nights, and holidays in 2026 and 2027, in contrast to a Health Minister who despises them, creates divisions, and has caused the largest medical strike in Madrid's history, we care for those who care for us,” stated Díaz Ayuso.
In concrete figures, a doctor will go from earning 424.83 euros for a weekday on-call shift performed in June of this year to 566.61 euros in 2028, representing a 33% increase. On holidays, the compensation will rise from 656.64 to 888.48 euros, a 35% increase during the same period.
Furthermore, starting in 2027, nearly five million euros annually will be allocated to increase compensation for extended working hours for SERMAS professionals. This measure will apply to all categories and will particularly affect SUMMA 112 doctors, nurses, nursing auxiliary care technicians (TCAE), and emergency medical technicians.
Díaz Ayuso also announced that the regional government has been working since February to ensure that 24-hour on-call shifts are no longer mandatory, through a “planned and viable transition.” “In contrast to the inaction of the Ministry, we will initiate the procedures to create the Technical Table of Doctors of Madrid as a stable dialogue forum, alongside the Community of Madrid Care Committee as a consultative and advisory body,” the president stated.
The Technical Table of Doctors of Madrid is envisioned as a stable working and dialogue body for physicians, while the Care Committee will have a consultative and advisory role for nursing professionals.
On another note, the Health Department will activate a Programme for Emotional Well-being and Mental Health for SERMAS professionals, which will include a peer support unit, as well as individual and group care.
According to data provided by the regional government, the public healthcare system in Madrid has created over 3,600 new positions since 2023, and in the first quarter of this year, more than 34,000 professionals will have consolidated their employment. The Community also highlights the promotion of extraordinary merit competitions without exams, the recovery of transfer competitions, the announcement of biennial public employment offers (OPE), and measures aimed at improving work-life balance.
