The public hospital Gregorio Marañón, in Madrid, has begun sending pharmacists to the homes of adult patients in Home Hospitalization. This is the first time this has been done in Spain and it has already reached nearly 60 people.
The pharmacists from the Gregorio Marañón Public Hospital, in Madrid, have been visiting the homes of adult patients enrolled in the Home Hospitalization program for a few weeks now. The initiative, which according to larazon.es is the first of its kind in Spain, has already reached nearly 60 people.
The professionals review the treatment in the patient's own home. They check what medications the patient is using, how they are administered, and what difficulties the patient encounters in their daily life. They also organise the medications and provide recommendations for safe management.
The program focuses on patients who need closer monitoring: polymedicated individuals, those with many changes in medication during their stay, or those who have difficulty administering their treatments. Before the visit, the pharmacist studies the medical history and prescription to prepare for the intervention.

During the visit, specialists also check for medications that may be confused with one another, drugs that are no longer necessary or have expired, and high-risk treatments, those whose incorrect use can have serious consequences. Additionally, they assess aspects such as pain control, sleep disturbances, constipation, or falls.
Afterwards, the pharmacist shares the collected information with the responsible team to consider possible adjustments and optimise the treatment. 67% of the interventions carried out are directly related to this face-to-face care.
Patients also have a direct contact phone line with hospital professionals available 24 hours a day to resolve queries, both during home hospitalization and upon discharge.
The Madrid centre has already conducted its first pharmaceutical care visits for paediatric patients and plans to gradually expand the program to more individuals in Home Hospitalization.
The project started after the Home Hospitalization Units received the reference accreditation from the Home Care Accreditation of the Joint Commission International (JCI) in 2025, the main body for certification and improvement of the quality of medical care.
In-person pharmaceutical care is activated when the care team considers that the pharmacist's intervention at home may be beneficial for the patient.
